Year-to-Date Information

1. Current Quarter Reports


• Submit Payroll Register for each payroll you've run in the current quarter.


2. Year-to-Date Reports
• Submit End of Quarter Y.T.D. for each quarter this year.

1. Employee Information

• Submit Master List from the last payroll of each quarter completed.
• Submit your most current ACH Report.

2. Company Banking Information

• Submit your most current Payroll Summary.

Submit Your Reports in One of These Three Ways:

Once you send your reports, a SurePayroll representative will be in contact with you to set up your first payroll.

1. Fax your reports to 847.676.5181. Be sure to include your phone number or e-mail address. You can print the attached cover sheet.
2. E-mail your reports to info@surepayroll.com.
3. FedEx your information to us for free. Call 1-877-954-7873 and we'll provide a shipping label and arrange to have your reports picked up at your location — on our dime.
