Know what to ask before signing
Payroll software for your dental practice needs to handle production-based pay for associates and hygienists, standard payroll for a mixed staff of clinical and administrative employees, and, if your practice is an S corporation, your own reasonable salary on the same platform. Confirm any payroll software handles all of it before you commit.
SurePayroll® By Paychex is built for small teams paying employees and contractors from the same platform.
What dental practices require from a payroll system
Production-based pay: Does the software calculate it for you, or do you calculate it manually and enter a number each period?
Mixed staff: Does one account support all employee types — associates, hygienists, office managers, and 1099 contractors — with year-end forms for each?
S-corp salary: Can you process your IRS-required reasonable salary on the same payroll run as your clinical and administrative staff?
A platform that requires a separate dashboard for employees and independent contractors adds unnecessary administration. Get clear answers before you evaluate pricing, features, and integrations.
Between 10% and 30% of employers misclassify at least some workers, according to Department of Labor-commissioned research published by the Economic Policy Institute.
SurePayroll calculates and files payroll taxes for practices running mixed pay structures: employees who receive W-2s, contractors who receive 1099-NECs from one dashboard.
How production-based pay works on a payroll platform
Production pay structures vary across dental practices. Some associates and dental hygienists earn production only. Others receive a base salary with a production bonus on top. The percentages, collection thresholds, and payment triggers differ by compensation structure. Many payroll platforms aren’t built to handle that variation.
Before you evaluate anything else, ask this: does your system calculate production-based compensation, or do you calculate it outside the system and enter a number each pay period?
Confirm the system supports both pay treatments (production-only and base-plus-production) and applies the correct withholding for each. If multiple associates or hygienists are on different percentages or collection thresholds, confirm the system can run all those configurations on one payroll run.
With SurePayroll, you enter the production amount each pay period, and SurePayroll applies the correct withholding and tax treatment from there.
The baseline features your practice needs
Any system you evaluate should include automated tax filings for federal, state, and local payroll taxes, produce tax forms for employees and independent contractors at year end, apply overtime pay calculations, and offer reporting and analytics. Verify these are standard inclusions, not add-ons.
Direct deposit should be standard and pay frequency flexible. You determine how often payroll runs and the service matches your schedule, not the other way around.
SurePayroll includes federal, state, and local tax filing, direct deposit, overtime calculations, and reporting, on every plan.
If your practice offers benefits, look for PTO tracking, benefits administration, and retirement plan deductions run through the system. If any staff member works across state lines or remotely, add multi-state capability to the list.
Find out whether the software integrates with the practice management software you already use, such as Dentrix, Eaglesoft, Carestream, or others. Name your system and ask directly. Ask what support looks like for a practice your size. Those answers tell you more about how a platform operates day-to-day than any feature list.
Payroll criteria for S-corp structured dental practices
If your practice is structured as an S corporation, you have one additional payroll requirement: your W-2 salary runs on the same account as your team's. As an active owner-employee, you pay yourself a reasonable salary before taking distributions. You process your salary the same way you process your clinical and administrative staff payroll, with the same tax calculations applied.
Confirm the service you’re evaluating supports your pay frequency as the owner-dentist independently from your staff's schedule, if the two differ. Some platforms handle this by default. Others require additional setup. Know which before you commit.
At year end, the service should generate your W-2 along with your team's W-2s and 1099-NECs. One system, one account, one year-end process.
Build your payroll setup on a system that fits your practice
Production pay, mixed staff classifications, and S-corp owner payroll, if that applies: confirm the payroll software handles what you need before you sign.
"I have been using SurePayroll for my small business for a few years now, and when it's time to expand in multiple locations, [I will] use them again."
